To Associate a Lookup Field with a Lookup table

Click the Add Lookup Field button.
In the new field Column, select a field name from the drop-down list.
Note: Once a field name is selected, it cannot be changed. If you selected the wrong name, delete the field and create a new one.
Click in the Lookup Table field and select a Lookup table from the list.
